LMC Templates: Patients moving into care homes

Gateshead and South Tyneside LMC have developed this guidance to clarify responsibility for patients moving into care homes across local practice boundaries.

It addresses a common area of uncertainty for practices and care homes. The guidance has been developed in collaboration with local commissioners, community services and GST LMC, and is intended to support consistent, safe and appropriate patient care.
