LMC Templates: Request to Complete Non-Contractual Shared Care Prescribing

Gateshead & South Tyneside LMC have developed a template for practices to use to respond to requests to complete non-contractual shared-care prescribing.

We would, of course, be delighted to be copied in to any such template letters sent, as long as patient details are redacted, in order that we can be kept aware of the extent of the issue.
