LMC Templates: Referrals and Advice & Guidance

Gateshead & South Tyneside LMC have developed a set of templates for practices to use if a patients referral has been rejected due to the use of an incorrect pro-forma. A template has also been provided for use when the referring clinician would like a patient to be referred directly without 'Advice & Guidance'.
We would, of course, be delighted to be copied in to any such template letters sent, as long as patient details are redacted, in order that we can be kept aware of the extent of the issue.
