Job summary


The successful applicant will work as an autonomous practitioner, responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure. The role can potentially be full or part time and based across our two sites in Washington and Sunderland.


Main duties of the job


  • Develop, implement and embed health promotion and well-being programmes

  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advise to patients

  • Implement and evaluate individual specialised treatment plans for chronic disease patients

  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

  • Undertake the collection of pathological specimens

  • Process and interpret pathology and other test results as required

  • Maintain accurate clinical records in conjunction with extant legislation

  • Maintain chronic disease registers

  • Develop, implement and embed an effective call/recall system

  • Be responsible for the implementation of an effective immunisation programme

  • Prioritise health issues and intervene appropriatelyRecognise, assess and refer patients presenting with mental health needs

  • Implement vaccination programmes for adults and children

  • Extended and supplementary prescriber, adhering to extant guidance

  • Contribute to practice targets (QOF etc.), complying with local and regional guidance

  • Support the clinical team with all safeguarding matters, in accordance with local and national policies

  • Support the team in dealing with clinical emergencies


About us


We are a merged practice with approximately 4300 patients. We have a clinical team comprising of GP Partner, a salaried GP, 2 long term locums, Practice nurse, HCA and non-clinical team including office manager/secretary and reception/administrators.

We are a small and friendly practice with strong work ethics.
